1. United Healthcare: United Healthcare has their headquarters in Minnetonka, Minnesota and just so happens to be the largest single healthcare carrier in the United States. There is an co-pay for this plan and it depends on what services you are receiving… generally they are around $10. Molina offers Medicare, Medicaid or a dual plan that includes both. The benefits that you will find with Molina include:
